We are a state certified Maternal Infant Health Program (MIHP).  Our mission is to aid Medicaid beneficiaries to address their needs and the needs of their infants in order to promote healthy pregnancies, positive birth outcomes, and infant health and development. 

We provide home visitation support and care coordination for pregnant women and infants who are Medicaid beneficiaries. Our services supplement regular prenatal / infant care and assisting healthcare providers in managing the beneficiary's health and well-being.

Our services are provided by a registered nurse and a licensed social worker.  The services provided are free of charge to the beneficiaries.


Our services include:

• Maternal and Infant health assessments (completed by nurse or social worker)
• Individualized plans of care based on client risks
• Coordination of MIHP services with client’s medical care provider and Medicaid Health Plan
• Client visits and interventions based on risk and priority
• Referrals for local community services
• Referrals to transportation services, as needed

The beneficiaries enrolled in our program receive:

• Risk Identifier and up to nine in home visits for mom
• Risk Identifier and up to nine in home visits for the infant and an additional nine with a physician’s order.    
• Substance exposed infants may also receive up to 18 additional in home visits.
• Assistance with childbirth/parenting education and transportation.
• Referrals to resources such as diaper banks, food pantries, clothing and housing.
